How to change a director's DOB or Place of Birth
To change the DOB or Place of birth of a director you need to fill out and lodge a 492 form and send it via Auspost.
A copy of the 492 form is available to download below.
How to lodge a 492 form:
- Complete the form.
- Clearly state which lodged form you are correcting (including the document number and date lodged). **
- Specify the mistake and provide the corrected information.
- Include supporting explanations and photocopies of a passport or ID.
- Ensure the form is signed by a director.
- Send the form via registered post to:
Australian Securities and Investments Commission
PO Box 4000
Gippsland Mail Centre VIC 3841

**The document number can be obtained from a search at ASIC Connect
How long does it take ASIC to process?
ASIC will review your correction request and:
- If accepted, they will update the public record. This can take up to 28 days to be completed.
- If rejected (e.g the error is too serious), ASIC may request further information.

Note: There is no fee to lodge a 492 form.
When can you use a 492 form?
You can use a 492 form to correct minor errors in a previously lodged form where the information submitted should have been correct at the time of lodgement.
Examples include:
- Typographical errors (eg wrong spelling of a name)
- Incorrect dates entered by mistake
- Small data-entry mistakes
- Administrative errors that don’t reflect an actual change in company information
Note: A 492 form is used to fix mistakes, not to notify of actual changes that occurred after the form was lodged.
When can you NOT use a 492 form?
You cannot use a 492 form if:
- You are updating information because the situation has changed (eg someone resigned or changed their address after the previous lodgement).
- The incorrect information was correct at the time but has since changed.
ASIC needs formal documentation to process the change.

In these cases, you must lodge a Form 484 – Change to Company Details (or another relevant ASIC form depending on the situation).
Below are some example scenarios of when to use which form:
